Annual Report on Graduate Programs for Fall 2012-Statistical Summary

Masters and PhD Advancement to Candidacy Forms

All master's students must file the advancement to candidacy application form no later than the first week of the quarter in which they intend to graduate.
PhD students must file the Report of Department Requirements Form when they nominate their oral qualifying exam committee.

General Petitions for Graduate Students      

290 Petition (Petition for Directed Studies) 

Petition for one on one independent study (not for research); for research use 297/299 units

Change in Major/Degree Objective

Used to change degree objective and/or major (can be used to add a major)

Filing Fee Petition-Masters Students

Used for master’s students to apply for Filing Fee status

Filing Fee Petition-PhD Students

Used for PhD students to apply for Filing Fee status

General Graduate Student Petition

Used to transfer or waive units or course work; extend time limit for the removal of Incomplete grades, etc.

Half-time Status and Reduced Fees App (for all except MBA students)

For those students who cannot attend more than half-time due to a full time job or for medical or personal reasons

Half-time Status and Reduced Fees App (for MBA students only)

For MBA students who cannot attend more than half-time due to a full time job or for medical or personal reasons

In-absentia Registration Application

For graduate students whose research or study requires them to remain outside California for the duration of a quarter

Intercampus Exchange Application

For those seeking to take classes at another University of California.  MUST HAVE ADOBE READER TO VIEW DOCUMENT.

Late or Retroactive Enrollment Adjustment Form

Used if adding or dropping units after the deadline

Leave of Absence (and/or Withdrawal) Form

Used for students to request from 1-3 quarters of leave from their studies

Permission to use Published Material in a Dissertation or Thesis

Used by students who want to use material that has already been published or has been accepted for publication

S/NC Petition (contact Graduate Division to see if you need this form)

Used for those seeking to enroll in an upper or lower division class on an S/NC basis

Withdrawal Form –use the Leave of Absence (and/or withdrawal) Form

Used by students who want to withdraw from the University
